If the MTO has asked for a vision report, or you're renewing or applying for a commercial licence, we measure your vision, complete the form, and submit the results.

Frequently asked questions

Yes. After the assessment, Dr. Fong fills out and signs the official vision form — whether it is an MTO driver’s licence / medical vision report or a vision form for a police or emergency-services application — confirming your results against the required standards.

Most vision assessments are straightforward and completed in a single visit. Bring your form and current eyewear, and let us know your deadline when you book so we can plan around it.

We see these assessments by appointment so the right testing time is reserved for you. You can book online or call (519) 622-1473.

For a non-commercial driver's licence, the MTO requires 20/50 vision with both eyes open and at least 120 degrees of horizontal peripheral vision. For a commercial licence, the requirement is 20/30. Police and emergency-services applications each set their own standards. Dr. Fong will confirm the exact thresholds for your form during your visit.

No. Driver and occupational vision assessments are considered third-party services, so they are not covered by OHIP. We can direct bill many extended health insurance plans on your behalf. If your plan does not cover it, the assessment is a self-pay service, and we will let you know the fee when you book.

A full occupational vision assessment: visual acuity, visual field, colour vision, stereopsis (depth perception), and a dilated fundus exam with retinal photography. It is built to meet the vision standards used by OACP and RCMP applications, and Dr. Fong completes and signs your form once the testing is done. Let us know your deadline when you book and we will plan around it.

Previous LASIK or other laser vision correction does not disqualify you from a police, fire, or paramedic application. Most services ask that you disclose the procedure on your form, but it does not count against your eligibility as long as your current vision meets the required standards.
