If the MTO has asked for a vision report, or you're renewing or applying for a commercial licence, we measure your vision, complete the form, and submit the results.

  • Your form or requirements package

    The MTO vision report or the vision form from the police / emergency service you are applying to.

  • Your deadline

    Let us know the date the form is due so we can plan the visit and any follow-up around it.

  • Current glasses or contacts

    Bring whatever you currently wear so we can record both your corrected and uncorrected vision.

  • Photo ID & health card

    For identity verification and to confirm any coverage that may apply.
